Abstract

Au Québec, la participation des usagers et des citoyens à la gestion publique constitue une composante essentielle de l’État et des formes de gouvernance qui caractérisent son action. Il faut remonter aux années 1960 et à la« Révolutiontranquille »pour en saisir toute la signification par rapport à la modernisation de l’État. Toutefois, au cours des dernières années, les choses ont changé. Le« modèlequébécois »des années 1970, ayant pour base la construction de l’État providence, n’a plus rien à voir avec celui des années 1990, soutenu par les valeurs néo-libérales et pratiquant le désengagement public. L’un des objectifs de cet ouvrage est de retracer l’évolution des formes institutionnelles de la participation des usagers/citoyens dans les secteurs de la santé et de l’éducation au Québec. Finalement, en mettant en lumière les nombreux registres à partir desquels les usagers et citoyens contribuent à l’amélioration des services en éducation et en santé, ce livre démontre sans contredit que l’avenir des systèmes de santé et d’éducation dépend d’une manière grandissante du rôle des principaux acteurs concernés.

Abstract

The effective management of appropriate health care should be able to contain medical care costs and improve accessibility while addressing rationing concerns. However, managed care in the United States has not lived up to the expectations set for it. Managed care quickly gained popularity among employers and public policy makers as a mechanism for curbing the excessive growth of health care insurance costs. Nonetheless, since its introduction, the system of largely for-profit managed care has been the subject of much public and political debate. The change from a fee-for-service system toward a system in which the health care insurance component is combined with the delivery of a broad range of integrated health care services for populations of plan enrollees that are financed prospectively from a limited budget has been widely criticized and has even been called repugnant. Instead of placing the blame on managed care organizations, however, we need to keep in mind that such organizations operate without societal agreement on critical issues such as a workable definition of health, an authoritative standard for defining the scope of entitlements, and on the distribution of labor between public and private sector entities. The health care system in the United States is also characterized by decentralization as well as the absence of a comprehensive health care planning or budgeting system, substantive access rules, and agreed-upon minimum health care benefit package. Therefore, managed care organizations only have limited responsibilities. The nonexistence of a shared, unifying paradigm of responsibility has been called the leading cause of the inability to manage health care appropriately. The stakeholders in health care operate on a set of widely varying interpretations of the notion of responsibility. The concept of genuine responsibility, recognizing the complexity of health care and the need for stakeholder-specific interpretations of responsibility, proposes as the underlying premise of responsibility (at least in regard to health care) the social agreement that distributive choices should be made on the basis of the premise of deliberate reciprocity. When all parties share the same foundation on which the notion of responsibility is built the resulting trust and cooperation among stakeholders enables them to find morally appropriate solutions in reforming health care. Abstract

Patient Flow: Reducing Delay in Healthcare Delivery is dedicated to improving healthcare through reducing the delays experienced by patients. One aspect of this goal is to improve the flow of patients, so that they do not experience unnecessary waits as they flow through a healthcare system. Another aspect is ensuring that services are closely synchronized with patterns of patient demand. Still another aspect is ensuring that ancillary services, such as housekeeping and transportation, are fully coordinate with direct patient care. It is the first book treatment to have reduction in patient delay as its sole focus, and therefore, provides the foundation by which hospitals can implement change. Reflecting the highly interdisciplinary and practitioner nature of this book, the chapters have been written by doctors, nurses, industrial engineers, system engineers and geographers, and thus, these perspectives provide the comprehensive view needed to address the problem of patient delay.

Abstract

In November 2005, New Orleans city leaders asked RAND to estimate the repopulation of the city in the aftermath of Hurricane Katrina. The Bring New Orleans Back Commission needed estimates of the city b2 ss population in the immediate future (the next three to six months) and the near-term future (the next one to three years) to guide the redevelopment planning process. The study was completed in early January 2006. A conceptual framework based on the costs and benefits of migration and on the role of social networks and physical constraints guided the estimates. Housing habitability was determined to be the key driver of the future population of New Orleans. RAND developed an approach to estimating future population for four points in time based on estimates of housing habitability, which were, in turn, determined by floodwater depth and the pace of housing reconstruction, as well as an estimate of the pre-Katrina population by the condition of its housing after Katrina. An important role for policymakers in shaping the repopulation process in New Orleans will be to minimize the uncertainty faced by residents and businesses by speeding up the reconstruction process.
